Transitional community rehabilitation

It is important to realise that recovery from brain injury can be lengthy journey. Hospital treatment often focusses predominately on therapy for an individual’s physical injuries with little attention being paid to the cognitive and functional aspects of brain injury, leaving the person physically recovered but unable to return to their home.

Transitional Community Rehabilitation provides step-down community based rehabilitation for those who have completed their physical rehabilitation but who continue to require cognitive and functional rehabilitation, including social and behavioural skills training.
