Referrals are welcome from the NHS, Clinical Commissioning Groups, Commissioning Support Units, health and social social care professionals, legal and case management professionals and families. Referrals are free of charge and without obligation.

Please contact us to arrange an assessment. Christchurch Group staff (including at least one clinician) will arrange to visit the referred individual to assess their needs and find out if they would benefit from a period of specialist neurological assessment and/or rehabilitation. You will be asked to provide copies of recent clinical reports and as much background detail as possible. In addition to providing rehabilitation, we also accommodate patients requiring intermediate care including re-enablement for up to 6 weeks.

Assessments will be conducted within five working days of receiving a formal referral. At this stage, clinicians also discuss rehabilitation goals with the family and review recent clinical reports to gather as much background information as possible. This enables them to make the most effective recommendations.Following an assessment and interdisciplinary team meeting, Christchurch Group will make recommendations via a formal written report. If a placement is recommended, then a comprehensive assessment report and bed offer will be sent with costs.
